Job summary

cape group is seeking a Disability Support Worker – Community Options to provide high quality community-based support for people with disabilities. You will implement support plans, assist with personal care, and promote independence and inclusion.

Applicants should have a Certificate III in Disability or Aged Care, NDIS experience, and a current Victorian Drivers Licence; Auslan is a plus. Multiple sites across Nunawading and Croydon offer casual hours with strong development opportunities.

About cape

cape group is a not-for-profit registered NDIS provider supporting people with disability across Melbourne's eastern and northern suburbs. We were formed in April 2025 through the merger of Nadrasca and Brite, bringing together more than 120 years of combined experience. Nepean Centre joined us in June 2026. Monkami Centre joined in July 2026.

The role

The Support Worker – Community Options is responsible for providing high quality support to people with disabilities in community-based settings. This includes assisting individuals to achieve their goals, promoting independence, and facilitating community participation. The role involves implementing individual support plans, providing personal care, and supporting skill development and social inclusion.

What you will do

• Uphold and advocate for the rights, dignity, privacy, and safety of people supported, ensuring protection from abuse, neglect, and harm in line with organisational policies and NDIS requirements.
• Provide personalised, respectful daily support, including personal care, health needs, medication administration, communication support, and emotional wellbeing.
• Support individuals to identify, plan, and achieve their personal goals through the development, implementation, and ongoing review of Personal and Weekly Plans.
• Facilitate meaningful activities that promote independence, community participation, valued social roles, and positive relationships.
• Maintain accurate, timely documentation and records, including care plans, evaluations, incident reports, and communication with families and support networks.
• Communicate effectively with participants, families, colleagues, and community stakeholders using appropriate communication methods and tools.
• Assist individuals to raise concerns and complaints, ensuring issues are addressed promptly and escalated when required.
• Contribute positively to a collaborative team environment, demonstrating professionalism, reliability, sound judgement, and proactive problem‑solving.
• Maintain safe, clean environments and comply with all workplace health and safety, emergency, and risk management procedures.
• Participate in mandatory training and professional development to maintain required competencies (e.g. First Aid, CPR, medication, manual handling).

Who we are looking for

Qualifications and Experience

• Minimum Certificate III in Disability or Aged Care
• Experience providing NDIS supports is essential
• A current Victorian Drivers Licence is mandatory.
• Well-developed computer skills (Microsoft Office, Internet and Outlook) and experience in using a Client Information Management System
• Auslan sign language (desirable but not essential)
