Job summary

ACCORD CARE is seeking an experienced Disability Support Worker to support a participant living in Albion, Victoria. The role involves personal care, mobility, mealtime safety, medication assistance and daily living activities in a high‑responsibility environment.

You will work with the participant, family and clinical staff, following participant‑specific support plans, swallowing guidelines and emergency procedures.

Qualifications

About the Role

We are seeking an experienced, reliable and compassionate Disability Support Worker to support a participant living in Albion, Victoria.

This is a high‑responsibility support role working with a participant who has cerebral palsy, a severe intellectual disability and dysphagia. The participant requires consistent and attentive support with personal care, mobility, mealtime safety, medication assistance, communication and daily living activities.

The successful candidate must be confident supporting complex physical and communication needs and be able to consistently follow participant‑specific support plans, clinical guidance, swallowing guidelines, manual‑handling procedures and emergency response protocols.

This position is suited to a calm, observant and safety‑focused support worker who understands the importance of structured routines, accurate documentation and continuity of care.

Key Responsibilities
  • Provide respectful personal care, including toileting, continence support, dressing and personal hygiene.
  • Provide safe mealtime assistance in accordance with dysphagia, choking and aspiration‑risk procedures.
  • Recognise signs of choking or aspiration and respond promptly in accordance with emergency procedures and current first‑aid practices.
  • Provide medication assistance in accordance with the participant’s support plan and ACCORD CARE procedures.
  • Perform one‑person ceiling‑hoist and sling transfers safely and in accordance with the participant’s manual‑handling plan.
  • Support the safe use of the participant’s powered wheelchair and shower commode.
  • Assist with positioning, mobility and pressure‑care requirements as directed.
  • Use non‑verbal and alternative communication methods to understand and support the participant’s choices, preferences and needs.
  • Follow Positive Behaviour Support strategies and use proactive early‑intervention approaches.
  • Complete clear and accurate progress notes, incident reports and handover documentation.
  • Follow emergency evacuation procedures for a non‑ambulant participant.
  • Maintain a safe, respectful and well‑organised support environment.
  • Work collaboratively with the participant, family, support team, clinical staff and allied health professionals.
  • Follow ACCORD CARE policies, NDIS Practice Standards and all participant‑specific support plans.
Skills and Experience
  • Previous experience supporting people with cerebral palsy, severe intellectual disability or complex support needs.
  • Experience supporting participants with dysphagia and a high choking or aspiration risk.
  • Demonstrated ability to follow Mealtime Management Plans and swallowing guidelines.
  • Confidence responding to choking incidents, medical concerns and other emergencies.
  • Experience providing medication assistance and completing medication records.
  • Experience using ceiling hoists, slings, powered wheelchairs and shower commodes.
  • Strong personal care, toileting and continence‑support skills.
  • Experience supporting people who use non‑verbal or alternative communication.
  • Understanding of Positive Behaviour Support and proactive early‑intervention strategies.
  • Strong observation, written communication and documentation skills.
  • A respectful, patient and person‑centred approach.
Mandatory Requirements
  • Certificate III or IV in Individual Support, Disability or a related qualification, or equivalent relevant experience.
  • Current NDIS Worker Screening Check.
  • Current Working with Children Check, where required.
  • Current First Aid and CPR certificates.
  • Current medication‑assistance training.
  • Current manual‑handling and hoist‑transfer training or demonstrated relevant experience.
  • Valid Australian driver licence.
  • Evidence of current Australian work rights.
  • Willingness to complete participant‑specific training and competency assessments before commencing independent shifts.
Desirable Experience
  • Dysphagia and mealtime‑management training.
  • Experience supporting a non‑ambulant participant.
  • Experience with emergency evacuation procedures for participants with restricted mobility.
  • Experience supporting people with complex or non‑verbal communication needs.
  • Experience working in Supported Independent Living or complex in‑home support environments.
  • Previous experience working for an NDIS provider.
