Job summary

Together Two is seeking experienced Disability Support Workers to join our team supporting participants with complex needs in Sydney's Inner West. You will deliver person-centred support, work with families and networks, and help participants progress toward their goals.

You will provide personal care with dignity, follow Behaviour Support Plans, maintain accurate notes, and collaborate with senior leads and allied health staff.

Qualifications

• Demonstrated experience supporting people with behaviours of concern, including following Behaviour Support Plans.
• Experience providing personal care.
• Full availability, including weekdays, evenings, weekends and public holidays.
• A current, full driver's licence and a reliable, registered and comprehensively insured vehicle.
• Strong communication skills and a calm, patient approach.
• NDIS Worker Screening Check (clearance).
• Working With Children Check (NSW).
• National Police Check.

Responsibilities

• Providing personal care with dignity and respect, including showering, toileting, dressing and continence support
• Supporting participants whose behaviours of concern are guided by a Behaviour Support Plan, using positive, proactive and least restrictive strategies
• Supporting participants to take part in community activities, appointments and daily living tasks
• Transporting participants safely to activities and appointments
• Writing clear, accurate case notes and reporting incidents in line with our procedures
• Working as part of a collaborative team with Senior Support Leads, allied health and behaviour support practitioners
