Job summary

Concept Care Disability Solutions is seeking experienced Disability Support Workers to provide one-on-one, in-home support to participants with complex needs in Sydney. Build trust, follow care plans, and document accurately while maintaining professional boundaries.

Qualifications

• Have at least two years’ full-time equivalent experience as a Disability Support Worker.

• Clear communication and accurate documentation.
• Understand professional boundaries, dignity and participant choice.
• Reliable, calm and confident in practice.

The Role

No two participants are the same. Depending on your experience, training and assessed competencies, you may support participants with:

• Personal care and daily living assistance.
• Complex and high-intensity supports.
• Medication support, accurate recording and escalation of concerns.
• Safe manual handling, transfers and mobility assistance.
• Meal preparation and domestic assistance.
• Community participation and appointments.

Most importantly, you’ll provide support that is safe, consistent, respectful and centred around the individual.

We’d love to hear from you if you:

• Have at least two years’ full-time equivalent experience as a Disability Support Worker.
• Have experience supporting people with complex care needs.
• Are confident following individual support and care plans.
• Know how to recognise and escalation changes or concerns.
• Communicate clearly and complete accurate documentation.
• Understand the importance of professional boundaries, dignity and participant choice.
• Are reliable, calm and confident in your practice.

You’ll also need a current:

• NDIS Worker Screening Check.
• Working with Children Check.
• National Police Check.
• First Aid and CPR certificates.
