Job summary

St Jude’s Health Care Group in Perth is seeking a Community Services Coordinator to lead a team delivering person-centered disability and community services from our West Perth office. You'll manage staffing and budgets, ensuring services meet client needs and standards.

You will maintain high-quality case notes, collaborate with clients and families, and drive improvements while adhering to NDIS requirements and regulatory guidelines. This is a full-time role based in Perth.

Qualifications

• Relevant qualifications/certifications or significant experience within the NDIS framework.
• Excellent documentation and reporting skills.
• Strong people skills and the ability to communicate effectively with a range of stakeholders.
• Must possess or be willing to obtain an NDIS screening clearance, a WWCC, a WA driver's licence and a reliable vehicle.

Responsibilities

• Working closely with clients to ensure that their funds and services are being used and delivered as effectively as possible.
• Confirming that support plans are being followed and that progress notes of a high quality are being kept.
• Managing and supporting and team of support workers.
• Ensuring that our services are compliant, in line with standards and that we are effectively managing risks.
