Job summary

Plena Healthcare is seeking a mobile Dietitian based in Perth to deliver high-quality nutrition care to adults in Residential Aged Care facilities and the local community.

This role requires travel across sites, availability on at least three weekdays (with preference for five), and collaboration with a supportive Dietetics Team Leader and colleagues. It offers access to resources and opportunities for professional development.

Qualifications

• Bachelor’s or Master’s Degree in Dietetics required.
• Current DA registration required.
• Australian working rights.
• Willingness to travel for a mobile role.
• Proof of current Flu Vaccination.

Responsibilities

• Provide person-centred dietetics care to clients.
• Help improve nutrition and hydration systems in Residential Aged Care homes.

• Complete menu and mealtime assessments.

Job description

Your Opportunity:

Reporting to your Dietetics Team Leader, you will provide high-quality mobile dietetics services to adults living in Residential Aged Care facilities and the local community. You will need to be based in Perth and available to work across at least 3 weekdays, with a preference for 5 days per week. Your responsibilities will include:

• Providing person centred dietetics care to all clients
• Helping to improve nutrition and hydration systems in residential aged care homes
• Delivery of educational sessions
• Completing menu and mealtime assessments
• Working closely with external stakeholders and your Plena colleagues to ensure the delivery of high quality dietetic care
• A commitment to continuous improvement and contemporary best practice
• Completing clinical documentation accurately and in a timely manner
• Contribute to team culture including but not limited to actioning the Plena values and actively engaging with dietetics colleagues

You will have access to extensive resources alongside internal clinical support. You will be presented with opportunities to attend our workshops and professional development opportunities.

To best support our customers, you will need:

• ABachelor’sorMastersDegree in Dietetics
• Current DA registration
• This is a mobile role and a willingness to travel is essential
• Willing to obtain NDIS workers screening and National Police Check
• Australian working rights
• Proof of current Flu Vaccination
