HammondCare is recruiting for a Dementia Support Coach to work within Dementia Support Australia (DSA), delivering hands-on, evidence-based dementia support and advice to clients, carers and care staff. The role is based in Osborne Park with a fixed term contract until June 2028 and involves travel as required.

Qualifications

• Strong organisational abilities and ability to prioritise in a busy work environment.
• Demonstrated commitment to excellence in customer service principles.
• Willingness to travel intrastate and interstate.
• Current driver’s licence and safe driving record.
• Qualification in Allied Health, Enrolled Nursing, Aged Care or related field preferred.

Responsabilités

• Collaborate with peers to upskill aged care staff and carers to support people with dementia.
• Support implementation of Dementia Support Australia plans on the ground with coaching techniques.
• Coordinate use of brokerage items with clients and carers to improve quality of life.
• Build rapport with staff and carers to foster confidence in dementia care practice.
