Job summary

Dementia Support Australia (DSA) is seeking a Dementia Support Coach to work with carers and staff across care settings, delivering hands-on dementia support and evidence-based advice. This fixed-term contract runs until June 2028 and is based in our Adelaide office.

You will collaborate with teams, implement plans, coordinate resources, and build trusted partnerships to improve quality of life for people living with dementia. Travel intrastate and interstate is required.

Qualifications

• Qualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable.

Responsibilities

• Work collaboratively with peers to assist in developing the skills, expertise and/or applied knowledge of aged care staff and carers, to better support people living with dementia.
• Support the implementation of the Dementia Support Australia plans on the ground, drawing on modelling and coaching techniques; while also seeking guidance from Team Leaders and Dementia Consultants as required.
• Manage and coordinate the use of brokerage items with clients and their carers to improve outcomes for quality of life for the person living with dementia.
• Act as a key liaison and trusted partner, when building rapport with staff and carers, resulting in individuals improving their confidence in dementia care practice.

This role sits within Dementia Support Australia (DSA), which is a national dementia support service, funded by the Australian Government, led by HammondCare. As a professional coach, you will work in diverse care settings, through the Hospital to Aged Care Dementia Support Program (HACDSP) – a service delivered by Dementia Support Australia. In this wholesome and holistic role, you can use your empathy, coaching skills and aged care experience; to deliver hands-on, evidence-based dementia support and advice to clients, carers and care staff.
