Calvary is seeking a Clinical Project Lead - Dementia Model of Care to drive implementation across hospital, aged care and home care services in NSW. You will work with consumers, families, clinicians and leaders to ensure consistent, person-centred dementia care and to embed the model of care.

The Clinical Project Lead - Dementia Model of Care will lead the implementation and evaluation of Calvary's Dementia Model of Care across hospital, residential aged care and home care services.
Working collaboratively with consumers, families, carers, clinicians and operational leaders, the role will support the delivery of consistent, person-centred and evidence-informed dementia care across Calvary services. The position will lead project activities, stakeholder engagement, education and capability-building initiatives to embed the model of care and improve outcomes and experiences for people living with dementia and their families.
The role will partner closely with service leaders and frontline teams to assess readiness, monitor implementation, address barriers, and drive continuous improvement and sustainable practice change.
