Job summary

Calvary Health Care invites applications for the Clinical Project Lead – Dementia Model of Care to lead the implementation and evaluation across hospital, residential aged care and home care services.

Working with consumers, families, carers and clinicians, you will drive person-centred, evidence-informed dementia care, oversee project activities, engagement and education, and partner with leaders to embed the model and sustain improvements across Calvary services.

Qualifications

• Relevant tertiary qualification in a health discipline (e.g. nursing, medicine, allied health).
• Experience coordinating and delivering healthcare or aged care improvement projects.
• Demonstrated understanding of dementia care and person-centred practice.
• Experience engaging with clinicians, leaders, consumers and families.

Responsibilities

• Lead implementation and continuous improvement of the Dementia Model of Care across services.
• Coordinate stakeholder engagement with people living with dementia, families, staff and multidisciplinary teams.
• Develop and manage implementation plans, governance, milestones and risk reporting.
• Assess readiness and support local adaptation with operational leaders.
• Build capability with education, resources and practical tools aligned to the model.
• Monitor outcomes and drive sustainable change using data and feedback.
