Job summary

HammondCare Horsley seeks a Dementia Consultant (Dementia Advisor) on a fixed-term, full-time basis. The role applies dementia-specific clinical knowledge in a residential aged care setting, mentoring care staff and implementing HammondCare's Philosophy of care and case management framework.

You will collaborate across teams, build capacity, and contribute to evidence-based care practices through supervision, evaluation, and reporting.

Qualifications

• 3 years of proven experience working with people living with Dementia.
• Registered Nurse qualification essential. Training and Assessment desirable.
• Strong teaching, mentoring and coaching skills.

Responsibilities

• On the ground dementia advisory services within HammondCare services.

About the role

A rewarding opportunity for a passionate Dementia professional to join HammondCare on an fixed term Full-time basis at our Horlsey Site.

Dementia Consultant, known within HammondCare Residential as a Dementia Advisor, focuses on improving the lives of individuals living with dementia by applying dementia-specific clinical knowledge and skills in a residential aged care setting. As a key member, you will mentor care staff to enhance their support and implement HammondCare's Philosophy of care and Case management framework. You will work closely with teams across the organisation, and build capacity, share expert knowledge, and contribute to the development of evidence-based care practices.

Some other responsibilities include:

• On the ground support. Providing dementia specific expert advisory services within HammondCare services, through participation in regular clinical review meetings
• Coaching staff, building their ability and capacity to deal with dementia related situations and escalations. Build staff capacity through communication, education, mentoring and coaching in practice; this can include spending time with the team on the ground and lead by example with care support, but also conducting case study discussions and review of documentations
• Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements
• Participate, contribute and learn as part of the Residential team in team meetings, professional supervision and team activities
• Evaluation and report by utilising HammondCare processes to communicate, monitor and manage work delegations and projects including evaluation and reporting
• Participating in projects and behaviour management planning

What do we need from you?

• 3 years of Proven experience working with people living with Dementia is essential
• Registered Nurse qualification essential. Further qualification in Training and Assessment is highly desirable
• Demonstrated skills in teaching, mentoring and coaching others
• Confidence with conducting care tasks such as personal care and resident general support
• High level of negotiation, communication and relationship building skills
• Problem solving and decision-making skills relevant to the aged and dementia care context
• Strong interpersonal and communications skills both written and verbal
• Strong administrative and computer skills using Microsoft Office products
• Ability to work independently and as part of a team
