Job summary

Sage Services Group is seeking an experienced Day Program Coordinator in Tamworth, NSW, for full-time engagement in aged care and disability support programs. You will plan, coordinate and deliver diverse day activities, partnering with support workers and families to promote social engagement and skills development.

You will lead person-centred activities, monitor outcomes, maintain compliant records and contribute to program improvements.

Qualifications

• Certificate III or higher qualification in Community Services, Disability Support, Aged Care or a related field.
• Minimum 2 years' experience in community services, aged care or disability support with group settings.
• Demonstrated experience in program coordination, activity facilitation or similar roles.
• Strong organisational and time management skills with the ability to manage multiple activities and priorities.
• Excellent communication and interpersonal skills with ability to engage diverse groups.
• Knowledge of legislation, standards and best practice in aged and disability services.
• Current Working With Children Check, Police Check and NDIS Clearance.
• Current First Aid and CPR.
• A valid driver’s licence and reliable access to transport.

Responsibilities

• Plan, develop and coordinate a diverse range of day activities and programs.
• Ensure activities are person-centred, engaging and aligned with goals and support plans.
• Liaise with participants, families and staff to understand needs and preferences.
• Manage program schedules, resources, equipment and materials.
• Facilitate group activities in a safe, inclusive environment.
• Monitor and document participant engagement, progress and outcomes.
• Maintain records and reports in line with regulatory requirements.
• Contribute to program improvement based on feedback and outcomes.
• Support induction and training of support workers involved in day programs.
