Job summary

Barwon Health is seeking a Connected Care Community Clinician to provide person-centred, home-based care coordination for clients at risk of hospitalisation. You will work with a multidisciplinary team across Barwon Health facilities and community services to deliver integrated care plans.

The role focuses on assessments, care planning, and safe transitions, requiring strong communication and collaboration with families, carers, and other health professionals.

Qualifications

• Extensive clinical experience in a relevant health field.
• Demonstrated ability to undertake comprehensive assessments and develop care plans.
• Advanced written and verbal communication skills; work within interprofessional teams.
• Proficiency with Microsoft Office and clinical databases.

Responsibilities

• Deliver person-centred, evidence-informed care coordination with consumers and carers.
• Collaborate with internal and external services to implement program priorities.
• Maintain professional competence, CPD and contribute to quality improvement.
• Keep accurate clinical documentation and comply with policies and safety standards.
