Job summary

Barwon Health is seeking a Connected Care Community Clinician to deliver person-centred care coordination. The role supports consumers in home-based, telehealth, or clinic settings, working within a multidisciplinary team to improve health independence and reduce unnecessary hospital care.

You will contribute to planning, documentation, and ongoing quality initiatives while maintaining professional standards and CPD requirements in a supportive environment.

Qualifications

About the role

The Connected Care Program aims to deliver better health outcomes and reduce hospitalisation rates by improved integration of care with existing services, providing specialist assessment, Client-led care plans, referrals & connections to relevant health and community services and early escalation pathways for clients. The service provides a targeted and individualised service to consumers with complex chronic health or psychosocial needs at risk of recurrent ED presentations, admissions and readmissions. The service is primarily delivered in the home, with telehealth, phone or in-clinic appointments at Barwon Health facilities as required.

The Barwon Health Connected Care Community Clinician will contribute to, and be part of the community-based, multidisciplinary team to support consumers and carers to achieve an optimum level of health independence through:

• Comprehensive, co-ordinated and integrated multidisciplinary services
• Provision of consumer–centred care to community members from a broad range of backgrounds
• Contribute to the development and delivery of high-quality, cost-effective community-based Care Coordination services
• Contribute to meeting the program key performance indicators (KPI’s)

Key responsibilities

• Deliver person-centred, evidence-informed care coordination, working collaboratively with consumers, families and carers to achieve care goals and ensure seamless transitions across health services.
• Work effectively within the Connected Care team and across internal and external services, contributing to meetings, service planning, communication and the implementation of program priorities.
• Maintain professional competence, registration and CPD requirements, support the development of colleagues and students, and actively participate in quality improvement and best-practice initiatives.
• Maintain accurate and timely clinical documentation and data, comply with Barwon Health policies, relevant legislation and health standards, and contribute to information management, OHS, risk management and patient safety.

What we’re looking for

• Extensive clinical experience in a relevant health field, with an understanding of chronic health conditions, complex needs and factors contributing to hospital admissions.
• Demonstrated ability to undertake comprehensive assessments and develop collaborative, person-centred care plans using self-management strategies.
• Advanced written and verbal communication skills, with demonstrated ability to work effectively within an interprofessional team and contribute positively to team culture.
• Demonstrated initiative and systematic approach to problem solving, with proficiency in Microsoft Office programs and clinical databases.
