Job summary

Responsibilities

• Contribute to meeting the program KPI’s.
• Deliver person-centred, evidence-informed care coordination with consumers, families and carers to achieve care goals and seamless transitions across health services.
• Maintain professional competence, registration and CPD requirements; support development of colleagues and students; participate in quality improvement initiatives.
• Maintain accurate clinical documentation and data; comply with Barwon Health policies, legislation and health standards; contribute to information management, OHS, risk management and patient safety.
