Qualifications

• Certificate III or IV in Aged Care/Disability/Community Services as a prerequisite.
• Diploma of Nursing with current AHPRA registration (Enrolled Nurse).
• Current NDIS Worker Screening Check.
• Current Working With Children Check.
• First Aid & CPR Certification.
• National Police Check.

Responsabilités

• Build and maintain professional, participant-focused relationships for a small group of participants.
• Understand each participant's preferences, routines, goals and aspirations.
• Coordinate and facilitate supports that enhance independence and quality of life.
• Act as a proactive problem solver to identify opportunities to improve outcomes.
• Provide a responsive, professional service focusing on participant experience.
• Collaborate with families, guardians, support coordinators, allied health professionals and internal teams.
• Assist with personal care, daily routines and activities of daily living with dignity and choice.
