Job summary

Primrose Quality Care is seeking a Disability Support Worker - High Intensity in NSW. The role focuses on assisting NDIS participants with daily living, personal care, community access and goal attainment.

You will work with clients who have complex support needs, including physical, cognitive or mental health challenges, delivering person-centred care and documenting progress with accuracy. Training and development opportunities are available within a collaborative team.

Qualifications

• Minimum 1 year of disability support experience.
• Current NSW Driver Licence with reliable car (Mandatory).
• Ability to drive clients when required.
• Current NDIS Worker Screening Check.
• Current WWCC where required.
• First Aid and CPR certification.
• Strong communication and documentation skills.
• Ability to work in a multidisciplinary team.
• Professional, reliable and respectful approach.
• Ongoing training and development.

Responsibilities

• Providing personal care and daily living support
• Supporting mobility, transfers and manual handling
• Using hoists and mobility equipment
• Assisting clients with wheelchairs and community access
• Transporting clients to GP and allied health appointments
• Assisting with household tasks and routines
• Following care plans and instructions
• Completing progress notes and documentation
• Communicating with ENs, RNs, CNCs and care team
