Responsibilities

• Person-centered planning: chat with client and family to understand goals.
• Complex physical care: mobility, transfers, positioning, daily tasks and devices.
• Medical supports: observe changes, follow plans, liaise with care team.
• Social time and participation: provide meaningful activities.
• Autonomy and advocacy: support decision-making and consent.
• Cultural humility: respect backgrounds and beliefs.
