Claro Disability Services in Melbourne is seeking a dedicated Support Worker to assist clients in accommodation across Southeast/Bayside Melbourne. You will work with individuals with complex needs to support daily living, mobility, medical plans, and community participation.

The role emphasizes person-centered planning, collaboration with families and clinicians, and culturally respectful care. Shifts are flexible, and the team offers guidance, induction, and ongoing support.

Qualifications

• Experience or interest in complex care, disability, and health nuances.
• Excellent communication and adaptability.
• Reliable, professional, and collaborative with good notetaking.

Responsibilities

• <p>Chat with the client (and family/advocates), get the know them and understanding of goals and routines.</p>
• <p>Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan</p>
• <p>Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.</p>
• <p>Social time and participation: Bring meaning, fun, and community access aligned with interests.</p>
• <p>Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.</p>
• <p>Cultural humility: Respect backgrounds, beliefs, and ways of being</p>
