Job summary

Harbourside Port Stephens is seeking a Personal Care Assistant to provide high-quality, person-centred care to Home Care clients in their own homes within Port Stephens. You will help with daily activities, promote dignity and independence, and work within care plans and standards.

The role requires Certificate III in Individual Support – Ageing (or equivalent), a valid driver's license, and current National Police Check.

Qualifications

• Excellent communication and interpersonal abilities to support residents and families.
• Ability to document resident information accurately and follow care plans.
• Effective teamwork with Registered Nurses and Care Partners in a home-care setting.
• Demonstrated commitment to dignity, independence and quality of life for clients.

Responsibilities

• Deliver person-centred care and support in clients' homes.
• Monitor wellbeing and report changes to the care team.
• Assist with activities of daily living per care plans.
• Maintain accurate client documentation and records.
