Connecting Families Disability Services Pty is seeking a compassionate Female Disability Support Worker for Isabella Plains, ACT. You will support a 33-year-old female with Schizoaffective Disorder, autism, C-PTSD, and alcohol use, helping maintain independence and engage with the community.

Role emphasizes trauma-informed, person-centred care, daily living support, and safe home routines within a multidisciplinary team. Strong communication and calm resilience are essential.

Qualifikationen

• Experience supporting individuals with psychosocial disabilities, autism, or mental health conditions.
• Strong communication and relationship-building skills.
• Calm, patient, and able to work effectively in challenging situations.
• Understanding of trauma-informed and person-centred support practices.
• Ability to identify and respond appropriately to early signs of distress.
• Able to work independently and collaboratively within a multidisciplinary support team.

Aufgaben

• Daily Living & Community Access support.
• Assist with daily living tasks and household activities.
• Support participation in community access, shopping, appointments, and social activities.
• Encourage engagement in hobbies such as art, photography, music, outdoor activities.
• Medication & health management: prompting, monitoring, and support per care plans.
• Promote medication compliance and report concerns to the care team.
• Nutrition & meal support: planning, shopping, and preparation.
• Monitor wellbeing during distress or increased alcohol use.
• Provide trauma-informed, autism-informed, respectful communication.
• Maintain predictable routines and provide reassurance in times of stress.
