Job summary

Claro Disability Services is seeking a dedicated Support Worker for community-based roles in the Brisbane region. You will work with clients with complex needs to support mobility, daily tasks, medical plans, and meaningful community activities.

You’ll team with families and clinicians, building relationships and adapting to changing schedules to help clients live more independent lives with dignity and respect.

Qualifications

• Person-centered, warm, patient and caring attitude.
• Interest or experience in complex care, disability and health nuances.
• Excellent communication skills and ability to read cues.
• Reliable and professional with good notetaking and teamwork.
• Flexible and adaptable to changing needs and shifts.

• Optional: manual handling or disability-support training.

Responsibilities

• Person-centered planning: chat with client and family to understand goals and routines.
• Provide complex physical care, mobility assistance, transfers and positioning as per plan.
• Support medical needs: monitor changes, follow plans, liaise with care team.
• Encourage social time and community participation aligned with interests.
• Support autonomy and advocacy, respecting consent and promoting independence.
• Respect cultural backgrounds and beliefs in daily care.
