Qualifications

• Experience in complex disability care and health supports.
• Excellent communication and teamwork.
• Flexible with shifts and able to adapt to changing needs.
• Empathetic and person-centered approach.

Responsibilities

• Provide person-centered planning with client, family, and advocates.
• Assist with mobility, transfers, positioning, daily tasks, and devices per plan.
• Monitor changes, follow clinical plans, and liaise with care team.
• Support social activities and community participation.
• Support decision-making, consent, and independence.
• Respect client cultural backgrounds and beliefs.
