Job summary

Connectlife Australia is seeking compassionate Disability Support Workers to join our team in Melton, VIC. This casual role offers immediate start and flexible hours to support personalised NDIS plans.

You will assist the person we support to build independence, participate in community activities and work with a team to achieve goals while respecting cultural preferences and privacy.

Qualifications

• Proven experience in disability support or related fields.
• Understanding of NDIS, person-centred practice, and individualised supports.
• Excellent communication and interpersonal skills.
• Reliable, punctual, flexible and professional approach.
• Ability to work independently and as part of a multidisciplinary team.
• Experience with complex needs or forensic disability is desirable but not essential.

Responsibilities

• Provide high-quality personal care and daily living support, including hygiene, dressing and grooming.
• Assist with community access and meaningful participation.
• Develop life skills and promote greater independence.
• Support participation in appointments, outings and activities aligned with goals.
• Maintain accurate records of support, incidents and progress.
• Communicate respectfully with the person, family and care team.
• Uphold rights, dignity, privacy and preferences under the NDIS plan.
• Assist with medication administration and adherence per the plan.
