Job summary

Hogan Care in Victoria, Australia is seeking a compassionate Disability Support Worker to join our Pascoe Vale South team. You will support clients with daily living, personal care, mobility, and participation in activities, while upholding person-centred, culturally safe care.

We value reliability, clear communication, and the ability to work both independently and as part of a team, including flexible shifts across days, evenings, weekends, and overnights when needed.

Qualifications

• Certificate III in Individual Support or similar required/preferred.
• Current First Aid and CPR certification.
• Valid driver’s license and access to a reliable vehicle (if required).
• Availability to work flexible hours including overnights, weekends and public holidays.

Responsibilities

• Support clients in achieving personal goals and promoting independence.
• Maintain accurate records and progress notes.
• Follow individualized care plans and report changes.
• Respond to emergencies or urgent situations as required.
• Assist with personal care (showering, dressing, grooming, toileting).
• Support daily living tasks including meals, cleaning and laundry.
• Help with mobility, transfers and use of equipment.
• Provide companionship and support community participation.
• Promote independence, dignity and wellbeing.
• Comply with policies, safety and quality standards.
• Assist with community access such as activities, clubs and appointments.
