Job summary

Qualifications

• Previous experience as a support worker or similar (preferred)
• Current First Aid and CPR certification

Responsibilities

• Support clients in achieving their personal goals and promoting their independence
• Maintain accurate records and reports of care provided, including progress notes
• Follow individualized care plans and report changes, and keep accurate records
• Respond to emergencies or urgent situations as required
• Assist with personal care (showering, dressing, grooming, toileting)
• Support daily living tasks including meals, cleaning, and laundry
• Help with mobility, transfers, and use of equipment
• Provide companionship and support community participation (shopping, outings, appointments)
• Promote independence, dignity, and wellbeing
• Comply with policies, safety, and quality standards
• Assist with community access support such as taking participants to outdoor activities, clubs and/or appointments
