Job summary

Hope and Change is hiring Disability Support Workers to join a growing team in Brisbane’s northern areas. You will provide in‑home disability support, including personal care, mobility and documentation, with a focus on safe, respectful, person‑centred outcomes.

We offer permanent hours across a rotating roster and above‑award remuneration where applicable, supported by an experienced, clinically informed leadership team.

Qualifications

• 12 months experience in disability, community or complex care.
• Certificate III or IV in Individual Support (Disability) or equivalent.
• Current First Aid and CPR certification, valid driver's licence.

Responsibilities

• Provide personal care and daily living support.
• Assist with community access and social participation.
• Support manual handling, mobility, medication administration and documentation.
• Follow individual support plans and behaviour support plans.

Skills

12 months experience

NDIS Worker Screening

Access to a reliable vehicle

Proficiency with mobile apps

Education
