True Diversity Pty Ltd is seeking compassionate and experienced support workers for Community Access and Self-Care roles in the Ipswich area. You will help participants engage with their communities, attend appointments, and develop daily living skills with person-centred care.

The role prioritises safety, respect and individualised support, with training provided. Availability across weekdays, evenings and weekends is valued as part of flexible shift coverage.

Formación

• Minimum 2 years of experience supporting people with a disability.
• NDIS Worker Screening Check.
• Working with Children Check (or willingness to obtain).
• Current Driver’s Licence and reliable transport, or willingness to use public transport.
• Certificate III in Disability Services (or equivalent qualification or experience).

Responsabilidades

• Support participants to access their communities and attend appointments.
• Assist with personal care, daily living activities, health and hygiene.
• Follow behaviour support plans and strategies, responding calmly, safely and consistently.
• Administer medication (where required) and maintain records.
• Drive participants safely to and from activities.
• Complete required documentation and reports on time.
• Follow all Work Health & Safety, NDIS Quality and Safeguards policies and procedures.
