Job summary

SA Health is seeking a Community Support Worker to provide individualised support to consumers in their homes and communities, following care plans and working with a multidisciplinary team to ensure safe, responsive, and person-centred care.

You will assist with personal care and practical tasks, collaborate with families and health professionals, and support consumer independence wherever possible within consumer-directed care principles.

Qualifications

• Demonstrate the ability to provide personal care and practical support, which may include showering, bathing, toileting, personal hygiene, meal preparation, cleaning and laundry.
• Work effectively with consumers, families, carers, health professionals and other service providers, demonstrating strong communication, reliability and confidentiality.
