Hatch Homecare Pty Ltd t/a Right at Home Sydney Northern Beaches is seeking an experienced Community Support Worker to join our team in Dee Why, NSW. You will assess client needs, develop support plans and help clients access health, welfare, housing, education and community resources.

The successful candidate will have a relevant qualification, at least 12 months in the field, and strong communication and organisational skills.

Formación

Certificate IV or higher qualification in Community Services, Individual Support, Disability, Mental Health or related field.
Minimum 12 months of relevant work experience in community services or disability services.
Excellent written, verbal and interpersonal communication skills.
Current Australian Driver Licence with access to a vehicle.
Current First Aid and CPR Certificate, or willingness to obtain.

Responsabilidades

Assess client needs and develop support plans to improve wellbeing and participation.
Coordinate access to health, welfare, housing, and employment services.
Record client information and maintain case notes and documentation.
Advocate for clients with government agencies and community organisations.
Promote community resources and support client participation in activities.
