Team Rehab Solutions is seeking an experienced Community Occupational Therapist to join our growing Sunshine Coast team in Gympie. You will deliver meaningful, community-based care, working in a collaborative multidisciplinary setting with flexible full-time or part-time hours.

You’ll provide functional assessments, home modifications, and ADL support while coordinating with NDIS, Aged Care, DVA, and other funding streams. A supportive team environment and growth opportunities await.

Qualifications

• Registered occupational therapist with current AHPRA status.
• Minimum 1–2 years’ experience preferred.
• NDIS experience valued but not essential.

Responsabilités

• Conduct functional assessments and ADL support.
• Develop capacity building programs in home and community settings.
• Prescription of home modifications and assistive technology.
• Coordinate care across NDIS, Aged Care, DVA and other funding streams.
• Work with a diverse community caseload including NDIS, Support at Home, DVA, Private Health, Workcover.
