Job summary

Burnie Brae Ltd. in Chermside, Brisbane, is seeking an experienced Registered Nurse for our Support at Home team. The role blends hands-on community nursing, care coordination and nurse leadership to ensure seamless client care across shifts.

You’ll deliver clinical assessments, medication management, and restorative care, while coordinating the team, managing schedules and escalating issues. The organisation fosters a supportive, inclusive culture and values person-centred, rights-based care.

Qualifications

• Bachelor of Nursing and current AHPRA registration as a Registered Nurse.
• Post-registration clinical experience of at least 2 years.
• Current First Aid and CPR certificate.
• Current Australian driver's licence and access to a vehicle for home visits.
• Current National Police Check and aged care worker screening.
• Strong organisation/time management and ability to switch between clinical, coordination and admin work.
• Excellent written and verbal communication skills.
• Commitment to person-centred, rights-based, culturally safe care.

Responsibilities

• Deliver community-based clinical care and develop nursing care plans for Support at Home clients.
• Undertake comprehensive clinical assessments including falls risk, pain, skin integrity, nutrition and cognition.
• Safely administer and manage medications and respond to clinical deterioration.
• Support restorative care, advance care planning and end-of-life care.
• Backfill Care Partner during planned/unplanned leave to maintain continuity of care.
• Coordinate nursing team scheduling, visits, assessments and leave cover.
• Provide clinical leadership and support to Support Workers.
• Monitor client budgets and escalate variances.
• Maintain documentation and compliance with Aged Care Act and standards.
• Identify and report incidents and contribute to audits and quality improvement.
