Job summary

Juniper is seeking a Community Registered Nurse to join our Home and Community Care team across the Perth metro area. You will provide clinical care and nursing support to clients living independently at home, conducting assessments, and developing care plans in collaboration with families and healthcare professionals.

We are looking for an AHPRA-registered RN with aged care or community care experience, capable of coordinating care, and able to travel between client homes with a valid driver’s

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Experience in aged care or community care.
• Strong clinical assessment and care coordination skills.
• Willingness to travel between client homes.

Responsibilities

• Conduct clinical assessments and respond to changes in client health.
• Develop, implement and review person-centred care plans with clients and families.
• Provide clinical care including medication management and wound care.
• Coordinate care with GPs and other health professionals.
• Maintain accurate clinical documentation and reporting.
• Support clients to maintain independence at home.
