Job summary

Pragma Home Care in Perth Metro and Peel region is seeking compassionate Registered Nurses to join our growing community nursing team. You will provide high-quality, person-centred care to clients in their homes, supporting health, independence and wellbeing.

You will perform assessments, administer medications, manage wounds, support chronic conditions, liaise with GPs, maintain documentation, and uphold infection control and safety standards.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Current National Police Clearance.
• Current First Aid and CPR.
• Current Australian Driver’s Licence and reliable vehicle.
• Community/home/aged care experience is highly regarded.

Responsibilities

• Conduct nursing assessments and monitor clients’ health and wellbeing.
• Medication administration and management.
• Wound care and dressing changes.
• Catheter, continence, diabetes and chronic disease support.
• Monitor vital signs and recognise changes in clients’ conditions.
• Escalate clinical concerns appropriately.
• Provide health education and support to clients and families.
• Maintain accurate and timely clinical documentation.
• Liaise with GPs and other healthcare providers.
• Follow infection control and workplace safety requirements.
