Job summary

The Singleton Community Health Nursing Team within Hunter New England Local Health District is seeking a motivated Registered Nurse to deliver wound care, acute assessments and chronic disease management in clients' homes and clinic settings.

You will use clinical judgment to assess changing health needs, develop care plans, educate patients and coordinate referrals, while collaborating with a multidisciplinary team. No afternoon or night shifts; full‑time, with flexible arrangements.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Current unrestricted NSW Driver Licence.
• Accredited Nurse Immuniser (NSW), or willingness to complete within 12 months.
• Experience managing varied caseloads independently.

Responsibilities

• Deliver wound care, acute nursing assessments and chronic disease management.
• Provide safe, person-centred care in clients’ homes and clinic settings.
• Monitor clients’ health, recognise deterioration and escalate concerns.
• Develop care plans, provide health education and coordinate referrals.
• Collaborate with the multidisciplinary team and support junior colleagues.
