Job summary

Reliance Community Services in Canberra is seeking experienced Disability Support Workers to provide person-centred support across daily living activities, community access, and skills development. You will work with participants to promote independence, dignity and inclusion, while following support plans and maintaining detailed records.

The role requires current NDIS Worker Screening, WWVP and First Aid/CPR, with availability across the week including some weekends.

Qualifications

• Experience as a Disability Support Worker or similar community services role.
• Excellent interpersonal and communication skills.
• Flexible with working hours, including some weekends.
• Right to work in Australia and current registrations/certifications.

Responsibilities

• Providing person-centred support based on participant needs, preferences and goals.
• Assisting with daily living activities, community access, social participation and skill development.
• Encouraging participant choice, independence and confidence.
• Building respectful relationships with participants, families and support networks.
• Following participant support plans, risk assessments and organisational procedures.
• Completing accurate progress notes and required documentation.
• Identifying changes in participant needs and communicating concerns.
