Job summary

Directions Health Services in NSW is seeking a dedicated Case Manager for the Pathways Murrumbidgee team based in Wagga Wagga. You will deliver person-centred, community-based AOD treatment and support to individuals and families affected by alcohol and other drug use.

You will perform holistic assessments, engage clients, and collaboratively develop individualized treatment plans. Regular travel across the NSW Murrumbidgee region is required, with opportunities to work across main office and

Qualifications

• Must have person-centred approach and strong communication skills.
• Understanding of AOD treatment options and harm minimisation.
• Ability to work with Aboriginal and Torres Strait Islander communities.

Responsibilities

• Undertake holistic assessments of clients seeking support for AOD use.
• Engage referred clients, explain treatment options and harm minimisation strategies.
• Develop individualised treatment and support plans with clients.
• Provide pre- and post-treatment support for residential/day programs.
• Deliver information, counselling and support to clients and families.
• Provide intensive case management for complex needs including co-occurring mental health concerns.
• Coordinate referrals with local services to ensure continuity of care.
• Undertake routine outcomes measurement and maintain client records.
