Job summary

Metro North Health is seeking a Clinical Pharmacist to deliver safe, patient-centred pharmacy services across multiple clinical specialties at Redcliffe Hospital, in collaboration with multidisciplinary teams.

Responsibilities

• Deliver clinical pharmacy services including dispensing, medication histories, reconciliation, and development of Medication Action Plans across inpatient, outpatient and discharge settings.
• Apply increasingly independent clinical judgement to complex decisions under supervision.
• Facilitate continuity of care through liaison with carers, medical staff, GPs, hospital and community pharmacies on admission and discharge.
• Work cooperatively within Pharmacy and multidisciplinary teams and with patients/clients.
• Identify, develop and implement quality and service improvement activities to enhance Redcliffe Hospital pharmacy services.

A permanent full-time opportunity to deliver high-quality clinical pharmacy services across a range of specialty areas, working closely with multidisciplinary teams to optimise medication management and improve patient outcomes.

About the role

As a Clinical Pharmacist, you will play a vital role in delivering safe, effective and patient-centred pharmacy services across a range of clinical specialties at Redcliffe Hospital. Working within a collaborative multidisciplinary environment, you will provide expert medication management, clinical advice and pharmaceutical care that supports positive patient outcomes throughout the continuum of care. This rotational position offers exposure to a diverse range of clinical areas, enabling you to further develop your clinical knowledge, decision-making capabilities and hospital pharmacy experience.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

• Deliver clinical pharmacy services [which may include but not limited to dispensing (inpatient, outpatient and discharge), requisitions, medication histories, medication reconciliation, development of Medication Action Plans, Pharmaceutical care Plans, TDM, drug information and counselling] to multiple Pharmacy work units in accordance with organisational goals.
• Apply increasingly independent clinical judgement to more complex clinical decision making under the clinical practice supervision or professional guidance of a more senior clinician.
• Facilitate the continuum of care through communication with carers, hospital medical staff, GPs, hospital and community pharmacies, nursing homes and case managers to ensure essential information is transferred completely and accurately between hospital and community settings on admission and discharge.
• Work cooperatively and effectively within the Pharmacy and multidisciplinary teams and with patients/clients
• Participate in the identification, development and implementation of quality and service improvement activities that enhance the delivery of pharmacy services within Redcliffe Hospital.
