Job summary

Bolton Clarke is seeking a Clinical Nurse Leader for the Southern Suburbs & Sutherland Shire in Sydney. The role is part-time and focuses on delivering high-quality, person-centred care to residents in our Belmore location.

You will report to the Care Manager and work with a team to complete assessments, care planning, and ongoing evaluation. AHPRA registration and aged care experience are preferred; strong communication with residents and families is essential.

Qualifications

• Previous experience as a Registered Nurse, preferably in aged care.
• Relevant qualifications and AHPRA registration.
• Registered Nurse experience and ability to communicate effectively.

Responsibilities

• Complete the initial assessment and care planning; ongoing management and evaluation of residents' care needs.
• Implement the nursing care plan and monitor residents per plan.
• Liaise with visiting medical officers and health professionals for planning and evaluation.
• Deliver holistic, person-centred care using the resident’s care plan.
• Supervise and maintain quality of care and documentation.
