Job summary

Metro North Health’s Caboolture Hospital is seeking a Clinical Nurse Discharge Planner to coordinate complex patient discharges and ensure safe transitions across acute, subacute, and community services.

You will apply advanced clinical assessment, plan and evaluate care, advise on community services, support families and work with multidisciplinary teams to improve discharge outcomes and reduce readmissions in a collaborative environment.

About the roleJoin the Patient Access and Clinical Effectiveness (PACE) team at Caboolture Hospital as a Clinical Nurse Discharge Planner, where you will play a pivotal role in coordinating complex patient discharges and ensuring seamless transitions across healthcare services. Using advanced clinical knowledge and assessment skills, you will work closely with patients, families, multidisciplinary teams and external service providers to facilitate safe, timely and patient-centred discharge outcomes.In this role, you will provide expert guidance regarding community care services, manage referrals for ongoing care, and support patients with complex healthcare needs.
