Job summary

Gold Coast Health, part of Queensland Government, seeks a Clinical Nurse Consultant to lead the Transition Care Team on the Gold Coast, delivering high-level nursing care and coordinating clients transitioning from hospital to home.

You will oversee a small team, conduct joint home visits, allocate caseloads, and drive quality service and professional development within a supportive public health environment.

Qualifications

• Bachelor of Nursing or equivalent.
• Registration with the Nursing Midwifery Board of Australia.
• Evidence of vaccination as required.
• Criminal History Screening is required.
