Job summary

Gold Coast Health is seeking a Nurse Practitioner (After Care Support Service) to join the Acute Care Treatment Team on the Gold Coast. The role involves advanced nursing interventions, teaching for diagnostic information, and coordinating clinical assessment with the health care team.

The successful candidate will have a Nurse Practitioner Masters, registration with AHPRA, and credentials in mental health nursing or be working towards it; a valid driver’s licence and vaccination status are

Qualifications

• Bachelor of Nursing or equivalent.
• Masters of Nursing (Nurse Practitioner) or recognised equivalent.
• Registration with the Nursing Midwifery Board of Australia.
• Credential in Mental Health Nursing or working towards credential.
• Driver Licence.
• Vaccination evidence and TB screening.
• Criminal History Screening mandatory.

Responsibilities

• Provide advanced nursing interventions with medicines initiation/titration/cessation.
• Apply teaching strategies for diagnostic information.
• Collaborate with health care team to achieve optimal consumer outcomes.
• Facilitate system and partnership integration to reduce fragmentation.

The Gold Coast University Hospital (GCUH) / Intake team's role is to be the first point of contact for the Gold Coast Hospital and Health Service Mental Health Service to provide triage, assessment, education, support, information, advice and referral. ACT Community is a mobile outreach crisis response team that offers a range of short-term options tailored to meet individual needs in the community which may also include acute care in the immediate post discharge phase of an inpatient admission.
