The Transition Care Program aims to provide a comprehensive multidisciplinary service to eligible clients over the age of 65 (55+ for Aboriginal and Torres Strait Islander peoples) to assist with the transition from hospital to home, prevent unnecessary admissions to residential care, and reduce hospital length of stay. The TCP nursing discipline's primary clinical purpose is to assess, support and educate clients with nursing needs and provide evidence based clinical care across a broad spectrum of chronic and acute conditions.
