Job summary

BlueCare is seeking a Clinical Nurse for Community Services in Townsville and Mackay regions. The role focuses on leading the frontline clinical team, providing mentorship, and delivering person-centred care to promote independence and quality of life for residents in their homes.

Applicants should hold a Bachelor of Nursing and active AHPRA registration with extensive nursing experience. Community funding knowledge is highly desirable.

Make a Difference Every Day as a Clinical Nurse in Community Services. BlueCare is embarking on an exciting transformation to strengthen the provision of quality nursing care in the community setting. Being part of our clinical nursing team within our neighbourhood structure, you will have a dedicated focus on providing nursing care and leadership, support and guidance to other care staff in the provision of quality care that promotes quality of life and enhances the ability of individuals to maintain their independence and remain within their home in the community.

Responsibilities

• Allocate a client portfolio and lead and inspire the frontline clinical team through guidance and mentorship.
• Hone into effective decision making and clinical experience to provide exceptional care whilst responding to clinical nursing issues and concerns.
• Review client care and care plans promoting independence & quality of life, making a real difference to the quality of care we deliver.
• Development, implementation, monitoring and review of care plans.
• Comprehensive and accurate assessment of residents.
• Assisting with the orientation of new staff and ongoing professional development.
• Support in ensuring the service meets all accreditation standards.

Requirements

• Bachelor of Nursing, or equivalent qualification.
• Current AHPRA registration.
• Extensive general nursing experience and demonstrated skills reflective of clinical experience.
• Ability to liaise with the care team and to provide co-ordinated care.
• Confidence to manage conflict, pressures and competing priorities.
• Experience working in a multidisciplinary team with ability to provide leadership, support and mentorship to other team members.
• Previous community nursing experience and knowledge in community funding is highly desirable.

You will be an experienced clinician, with considerable experience within an acute or aged care environment. You’ll be an advocate for person‑centred care, leading by example through exceptional communication and customer service.
