ONCALL | SACARE is seeking a Part Time Clinical Case Manager to support people living with disability across Supported Independent Living facilities. The role is a leadership position driving continuous improvement in clinical care and service delivery.

Responsibilities include developing and reviewing care plans, guiding a multi-disciplinary team, and ensuring compliance with care standards and regulatory requirements.

Qualifications

• Nurse registration with AHPRA is required.
• Experience in disability care is essential.
• Strong administrative and organisational skills.
• Ability to liaise with multidisciplinary teams and stakeholders.
• Commitment to quality clinical care and continuous improvement.

Responsibilities

• Lead clinical care plan development and review for clients.
• Assess and develop care plans for new clients, including Complex Care Pathway cases.
• Support clients to set goals within the Peace of Mind framework.
• Maintain up-to-date knowledge of clinical care best practices.
• Promote high-quality service and act as a positive role model for staff.
• Ensure incidents are reported with proper risk assessments.
• Comply with applicable care delivery regulations and staff awareness.
• Maintain knowledge of NDIS High Intensity Daily Personal Activities requirements.
• Collaborate with General Manager and Clinical Nurse on care decisions and development.
• Support Learning & Development related to client care decisions.

We are looking for a Part Time Clinical Case Manager to join our expanding team. Specialising in working with people living with a disability, the successful applicant will contribute as a key member of our multi-disciplinary team.

This is a unique leadership opportunity for an experienced disability, health or community services professional capable of driving a continuous improvement culture and practice across the organisation.

Based across our Supported Independent Living Facilities, the successful candidate will be responsible but not limited to:

• Ensures that client health and well-being, capabilities, independence, choice, privacy, dignity, and safety are maintained and enhanced through the provision of quality clinical care plan development and review
• Appropriate assessment & development of care plans for new clients including those from Complex Care Pathway care plans
• Support individuals to formulate goals and aspirations within the Peace of Mind Framework
• Maintains a current knowledge and best practice in clinical care and lifestyle service delivery.
• Strives to improve the quality of service provided within SACARE.
• Acts as a positive role model to nursing and support staff.
• Ensures that all incidents are reported promptly and accurately with the appropriate risk assessment procedures undertaken
• Ensures that all relevant legislative requirements relating to care delivery are always complied with and ensures facility staff have awareness of those requirements.
• Maintain a current knowledge of the NDIS High Intensity Daily Personal Activities requirements
• Liaise with the General Manager –Accommodation Services and Clinical Nurse
• Learning & Development as it relates to decisions affecting client care & clinical needs.

To be considered for the role, candidates need to have:

• Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• Manual handling certificate
• First Aid certificate
• NDIS Workers Check
• Working with Children's Check
• Clinical expertise in the management of medical conditions related to disability care
• Experience working in disability care
• Excellent administrative, time management and organisational skills
• Demonstrated positive and flexible attitude with a willingness to take on a wide range of diverse tasks and adjust to competing priorities.
• Excellent interpersonal and communication skills with the ability to work with individuals from all socio-economic and cultural backgrounds;
• Sound ability to use the Microsoft Office suite of products.
