Job summary

Mercy Community is seeking an experienced APHRA Clinical Care Partner to join our Community Aged Care Services team in Rockhampton on a permanent full-time basis. This role delivers care management enabling older people to live safely and independently at home within a rights-based, person-centred framework.

You will collaborate with participants, their supporters and interdisciplinary teams to coordinate tailored support, with clinical oversight and care planning responsibilities.

Qualifications

• Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent.
• Experience in care coordination, case management or similar roles.
• Strong communication, empathy and cultural competence.
• APHRA registered qualification in Nursing or Allied health.

Responsibilities

• Collaborate with participants to develop individualized care plans within required funding timeframes.
• Incorporate the participant’s goals and environmental requirements within the plan and delivery.
• Establish and update care plans and funding requirements, ensuring participants understand available services and financial responsibilities.
• Review care notes and service delivery data.
• Provide clinical oversight and clinical care management to clients under support at home.

About the role

Mercy Community are on the search to find an experienced APHRA Clinical Care Partner to join our Community Aged Care Services team based in Rockhamptonon a Permanent Full-time basis.

The Care Partner is the designated person under Community Care, responsible for delivering care management that enables older people to live safely and independently at home. Working within a rights-based, person-centred framework, the Care Partner collaborates with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive and inclusive support.

This role involves but not limited to -

About you

As the successful applicant, you will possess -

• Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent
• Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services
• Strong communication, empathy and cultural competence.
• Skilled in using care management or CRM systems
• APHRA registered qualification in Nursing or Allied health
