Job summary

Respect Aged Care in NSW seeks a Clinical Care Coordinator to lead a nursing team in a purpose-built residential aged care home. You will manage admissions, develop person-centred care plans and mentor staff while delivering high-quality clinical care and ensuring regulatory compliance.

The role offers opportunities to mentor, advance care planning, and collaborate with families and management to improve resident outcomes in a supportive, values-driven environment.

Qualifications

  • Bachelor of Nursing with current AHPRA registration.
  • Minimum 2 years’ nursing experience in residential aged care.
  • Demonstrated ability to work collaboratively within multidisciplinary care teams.
  • Strong clinical assessment, documentation, and evidence-based practice skills.
  • Excellent communication and interpersonal skills to engage residents, families and colleagues.

Responsibilities

  • Coordinate admission of new residents with comprehensive assessments and person-centred care plans.
  • Lead, mentor and support the care team to deliver safe, high-quality clinical care.
  • Provide hands-on nursing care including medication administration and wound management.
  • Maintain accurate clinical documentation and ensure infection control practices.
  • Communicate effectively with residents, families and external healthcare providers; drive quality improvement.

No two days are the same! As Clinical Care Coordinator you will:
  • Coordinate the admission of new residents, completing comprehensive assessments and developing person-centred care plans in collaboration with healthcare professionals.

  • Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.

  • Provide hands‑on nursing care, including medication administration, wound management, and ongoing clinical monitoring.

  • Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.

  • Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.

About You

You’ll thrive with us if you have:

  • Bachelor of Nursing with current AHPRA registration.

  • Minimum 2 years’ nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.

  • Demonstrated ability to work collaboratively within multidisciplinary care teams.

  • Strong clinical assessment, documentation, and evidence-based practice skills.

  • Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.

  • Proficient computer and information management skills, including Microsoft applications and clinical software systems.

  • Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
