About the Role:

As a Clinical Care Coordinator, you'll lead our clinical and care teams to ensure residents receive exceptional care while nurturing an understanding of their unique identities and experiences. You'll also play a pivotal role in our homes clinical governance and may manage specialized clinical portfolios, such as education, palliative care, and infection control.

Key Responsibilities:

• Ensure comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident's needs by our clinical staff.
• Prioritize residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
• Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
• Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
• Conduct routine clinical assessments to promptly address identified gaps in care.

About you:

• Currently registered as a Nurse with AHPRA, with no restrictions.
• Demonstrated senior management experience in the aged care sector.
• In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
• Proficiency in managing human resource matters, complemented by strong negotiation skills.
• Skilled in utilizing various information technology tools and software, including Word and Excel, with a quick ability to adapt to new skills as required.
