Job summary

Calvary is seeking a Clinical Care Coordinator to lead the care team and ensure high-quality clinical services in a resident-centered aged care setting in The Mariner, South West Rocks.

The role requires an AHPRA-registered RN, strong communication skills, and the ability to work with a multi-disciplinary team to deliver compassionate, compliant care and effective care planning. A supportive environment and professional development opportunities are offered.

Qualifications

• RN registered with AHPRA and eligible to practise in Australia.
• Strong communication and interpersonal skills for multi-disciplinary teams.
• Ability to interpret and apply aged care legislation and funding models.
• Commitment to contemporary aged care nursing practice and resident-centered care.
• Proven ability to lead care planning and coordinate clinical services.

• About the role

  As the Clinical Care Coordinator you will coordinate and lead the care team in consultation with the Home Manager. You will ensure compliance with legislative and statutory requirements to enable us to continue to deliver a high standard of care and most importantly, a quality lifestyle for our residents.

  Working within a supportive and dynamic team, you will be provided with a challenging and diverse work environment, as well as an opportunity for you to contribute and make a real difference to the residential aged care community.

  We currently have a full time permanent opportunity for an experienced Clinical Care Coordinator for our facility in The Mariner, South West Rocks.

  Your main duties will include:

  • Coordinate the delivery of quality clinical services in line with a resident centred model of care
  • Maintain current knowledge and understanding of Aged Care Funding models
  • Manage and provide efficient, effective and compassionate care planning and assessment
  • Coordinate the availability of medical stock and other health related supplies to ensure sufficient and adequate supplies

  About you

  • Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
