My Guardian is seeking a motivated Client Intake Officer in Hobart to lead face-to-face engagements with prospective clients and families, guiding them through intake and decision-making with empathy. You will work closely with Tasmania BDMs and Case Management, coordinate onboarding, explain funding and services, and maintain accurate CRM records to support regional growth.

Qualificações

• Minimum 1 year of client intake, case management, or customer-facing experience in a similar sector.
• Knowledge of Aged Care (Support at Home / Home Care Packages) funding and NDIS frameworks.
• Excellent interpersonal and communication skills.
• Ability to engage face-to-face with clients and families.
• Comfort working in a fast-paced, target-driven environment.

Responsabilidades

• Meet prospective clients face-to-face and assess needs to recommend care solutions.
• Explain SaH Packages and NDIS services, converting enquiries to clients.
• Collaborate with the TAS BDM team and Case Management for care delivery.
• Coordinate onboarding, documentation, waitlist, and referrals.
• Maintain CRM records and report on regional client acquisition and conversion.
• Support rostering and clinical teams to align care with funded packages.
